Who may avail of the service?
All bona fide resident of Guimba, Nueva Ecija within the existing service
line.
Schedule of availability of service:
Monday – Friday
8:00 AM – 12:00 Noon
1:00 PM – 5:00 PM
ABOUT THE SERVICE: The connection will not be made until it is inspected and duly signed by the official plumber who conducted the inspection.
HOW TO AVAIL OF THE SERVICE:
Step I – Secure application form from:
a.) Diane Barawid & Evelita Javier - Customer Service
b.) Rommel G. Gragasin – Engineering Assistant
Step II – Accomplished the form and submit ready for inspection
Step III – Inspection be conducted within 1 – 3 days by the official plumber of the district
Step IV – Payment of charges to the cashier
a.) Within the service line = P 2,700.00 plus materials if any
b.) Boring + 10 meters P.E. ¾ = P 4,050.00 plus materials if any
DURATION:
Tapping within the service line: 2-5 working days Boring : 3-10 working days

With Notice: Directly proceed to the cashier for the issuance of the Official receipt
Without Notice:
a) Proceed to the Management Information System Analyst (Susana G. Barawid) for the total amount due
b) Proceed to the cashier for the issuance of the official receipt
ABOUT THE SERVICE:
Pay on time to avoid additional 10% penalty after due date and to avoid disconnection
REQUEST FOR SERVICE CONNECTION

Reconnection Fee and cost of material must be paid before reconnection of disconnected service connection
